Agency Documents & Forms

  • Individuals seeking their own reports may submit a written request specifying the records sought.
  • Insurance companies / attorneys seeking reports should submit a written request on company letterhead indicating your involvement and/or the party/parties represented and specify the records sought.
  • Agencies seeking reports should submit a written request on agency letterhead indicating your involvement and specify the records sought.

Requests may be submitted via:

  • In person at the address above

Please note that requests are processed in the order in which they are received.

If you would like to make a request for public records under the Access to Public Records Act (APRA) for documents maintained by the Division of Sheriffs, please complete the Public Records Request Form below.
